When Holston Medical Group implemented the Precision.BI solution, it was because, “We are a patient- driven organization and we need/use the information provided by Precision.BI to make decisions,” according to Executive Director Craig M. Kilgore.

He adds, "For many years, HMG used a product which, at the time, was considered "best in class" for data extraction and reporting. After determining this product no longer met our needs, due largely to the varied nature of data sources, we made the decision to purchase another product. After evaluating several products, we selected Precision.BI, which is also sold by Allscripts under the TouchWorks Analytics name, as our data extraction and reporting product. We continue to be impressed with the power and flexibility the product has and would make the same decision again today."

HMG has implemented many projects that have provided great improvements within the practice.

Dr. Steve Adkins-Chairman of the Holston Medical Group EHR Committee states: “The biggest benefit has been the development of dashboards. We now have physician dashboards going to individual doctors where they can see their own metrics and comparative data. It is relevant information that we want to follow for quality improvement. It has eliminated physician practice silos. As we know, physicians by nature, are very competitive and don’t want to be outliers. They change their behavior because they know the data is accurate, validated and certified. Improvements are made. There has been an impressive result since we now have the ability to combine sources of data from our practice management and clinical system.”

As an example of the physician dashboard, the following chart demonstrates how the physicians can track their progress against the practice’s e-prescribe initiatives. They can track month over month how many scripts are faxed, phoned, printed or e-prescribed. This is an effort to reduce internal operation costs and prevent potential pharmaceutical errors.

 

In addition to the individual physician dashboards, the CEO requested a dashboard to be available when he performs individual annual physician reviews. With this dashboard he is able to sit down with the provider on a regular basis without being dependent on the IT staff to provide the reports for him. As an example, one of the areas that is being closely monitored is whether or not physicians are referring to specialists within the practice. The chart below tracks which top ten providers are referring patients to a specific specialist.

 

The CEO is also keeping a close eye on charges and payments by physician. Within the dashboard he can track trends, spikes and dips. This allows a conversation during the review to identify areas of improvement in their practice and processes.

Bob Salyer, Manager, HMG Decision Support, explains,“After the solution was installed, the demands for reports quadrupled because now the end-users could get reports and analysis they could never get before. When they started seeing the data that could be produced from the Precision.BI solution it was like a storm. They wanted more and more reports.”

As a result they have various projects running simultaneously, such as:

• Ensuring that all pediatric patient immunizations are completed. An analysis is performed and provided to the physicians for their patients as a quick reference prior to the patient encounter.

• Tracking the specific time period between Gardasil vaccinations. Immunization processes are monitored against appointments to ensure that each series is complete within the time required.

• Identifying the last physicals for patients in specific age demographic ranges and contacting the patient to schedule the physical if it is due.

• Running a monthly analysis that looks for a specific appointment type. There is then a follow up with a phone call in addition to their automated phone system. This has reduced the number of “no-shows.”

• Watching patients that are in need of regular mammograms and following up with the patient via a phone call to ensure that they are scheduled for their mammogram.

• Tracking how many hours of nurse time one practice had compared to another. Here they can slice the data and determine how many hours of nurse care are associated with each visit to find areas of improvement.

“We didn’t have this at all before PBI. We are improving productivity and patient care,” Bob continues. “Now they can answer the ‘why’s’ because of the data and that is what makes things more efficient.”

Dr. Steve Adkins adds, “We have been utilizing electronic medical records for many years. We have even consulted with Allscripts customers and prospects. When an organization implements an electronic medical record everyone is focused on data entry and getting records stored in EMR. EMR is a valuable tool and will hopefully help you manage your data. But the real value of the EMR is the accumulative information in the database. You only start using EMR when you start extracting the information in an analysis format. When you start putting analytical tools such as Precision.BI on it you realize the full potential of the EMR.”
